Enter the district where your business is located, for example, Central West End or Kingsway Business District.
If more than one person owns the business, enter the primary contact person.
Select, 'yes' or 'no.' A new business is a business that is has been open for less than a year at that location or a business that will open in the near future.
Enter the number of years your business has been open. Select, 'N/A,' if your business has been open less than a year.
Enter the number of months your business has been open. Select, 'N/A,' if your business is a new business that has not opened.
If you are a sole proprietor, or the only worker, enter 1. If your business works with independent contractors, enter the number of contractors that work 40 or more hours per week.
If your business works with part-time independent contractors, enter the number of contractors that work less than 40 hours per week.
If the property is owned by more than one person, please enter the primary contact person. If the property is owned by an entity (LLC, Corporation), please enter the the primary contact person.

The funding of the Facade Program is financed wholly through a grant from the Department of Housing and Urban Development and the Community Development Administration under the provision of Title I of the Housing and Community Development Act of 1974 (Pub L, 93-383 42 USC 5301 et seq.).
