2015 Medical and Dental Open Enrollment Period

Open Enrollment for Medical and Dental Insurance will run from April 27 through May 22, 2015 with an effective date of June 14, 2015. Deductions will be made on paychecks dated July 2, 2015.
UnitedHealthcare will offer Choice Plus, Navigate and a third option which is a High Deductible Health Plan. Please read your open enrollment material for information regarding this new option.
We will continue to offer Dental Source and United Concordia dental insurance. The rates for Dental source will remain the same but United Concordia rates have increased slightly. If you want to cancel coverage in either plan, you MUST submit your request during the open enrollment period.
Employees do not have to complete a medical enrollment form this year unless they are making a change. Supporting documentation must be submitted as well as Social Security numbers for any new dependents.
New identification cards will be issued to all employees who are enrolled due to plan changes.
Please contact your payroll clerk if you have not received your open enrollment information. All forms must be submitted to the Employee Benefits Section of the Department of Personnel no later than 5:00 p.m. on Friday, May 22, 2015.
