Obtain Affordable Care Act 1095 Forms
ACA 1095-B and C forms are sent out by the City and the insurance carrier. Find out how to request additional copies.
Overview
The Affordable Care Act of 2010 requires plan sponsors like the City of Saint Louis and also insurers, like Anthem BCBS to report health information and participant coverage data to the Internal Revenue Service. The reporting is required annually and is done early in the year for the previous calendar year.
The City issues form 1095-C to employees to the address of record in the City system. Anthem BCBS, as the medical carrier for the City, issues 1095-B forms to members via mail to the member’s address of record.
The 1095-C form will not display dependents’ names but the 1095-B form will.
It is not necessary to wait for Forms 1095-B or 1095-C in order to file your taxes. While the information on these forms may assist in preparing a return, they are not required. More information from the IRS about Form 1095.
