Open enrollment for medical and dental plans is from May 8 to May 19, 2017.  NO MEDICAL OR DENTAL ENROLLMENT FORM IS NEEDED THIS YEAR IF YOU WANT TO CONTINUE IN YOUR CURRENT LEVEL OF COVERAGE.

If adding dependents not currently enrolled, you must provide supporting documentation and Social Security numbers.  The types of supporting documentation are included in the brochure that has been distributed by the payroll clerks to City employees.

If dropping dependents, you must complete the proper Change Form for medical and/or dental.

The effective date of coverage will be June 25, 2017 and new deductions will be made on your paycheck dated July 14, 2017.

Express Scripts prescription drug coverage will not change.

Please contact your payroll clerk if you need to enroll, add dependents or cancel dental coverage.

Nearly $300 billion a year is spent to combat the effects of smoking, and one in two Americans who smoke die from smoking.  Smoking is the #1 preventable cause of death in the United States.  Smoking is also one of the leading reasons that health insurance costs rise at unsustainable rates.  Therefore, Smoking Cessation Programs will be scheduled throughout the year, and employees will be notified of the dates, time and locations for the classes.
