Overview

You can use this form to commend a first responder who you feel has performed exceptionally in their job duties or in a response to a request for assistance.

Preparation

Prepare to submit the following information:

  1. The name of the first responder.
  2. Their role (police, fire, EMS, or a City resident who has contributed to public safety).
  3. The reason you are submitting them for commendation.
  4. You can submit the form anonymously, or you may provide your first and last name, email address and/or phone number.

What to Expect

The first responder will be informed that you commended them. They will see only your name and the reason you commended them.
