Overview
You can use this form to commend a first responder who you feel has performed exceptionally in their job duties or in a response to a request for assistance.
Preparation
Prepare to submit the following information:
- The name of the first responder.
- Their role (police, fire, EMS, or a City resident who has contributed to public safety).
- The reason you are submitting them for commendation.
- You can submit the form anonymously, or you may provide your first and last name, email address and/or phone number.

What to Expect
The first responder will be informed that you commended them. They will see only your name and the reason you commended them.
