Requirements to Obtain a Gateway ID Card
Items needed to apply for the card.
Primary or Secondary Forms of ID
To apply for the Gateway ID, you will need one item from the list of primary forms of ID, OR two items from the list of secondary forms of ID.
Primary Forms of ID – One Required
- A current U.S. or foreign passport (including U.S. passport card)
- State-issued driver’s license
- State-issued identification card
- U.S. military identification card that includes a photograph.
- U.S. Permanent Resident card (Green Card)
- Current visa issued by a government agency
- Consular identification card
- Photo identification or driver’s license card with name, address, date of birth, and expiration date issued by another country to its citizens or nationals as an alternative to a passport for re-entry to the issuing country
- National identification card with photo, name, address, date of birth, and expiration date
- School ID card with photograph
- Voter registration card
- U.S. Military ID card or draft record
- Military dependent ID card
- U.S. Coast Guard Merchant Mariner Card
- Native American tribal document
- Letter from a government or social service agency attesting to the identity of the applicant
- Missouri Dept. of Revenue form 5862, Certification of Homeless Status and Residency
Secondary Forms of ID – Two Required
- A W-2 form issued within the last year
- A signed Social Security Card
- Original or certified copy of birth certificate
- U.S. military discharge document (DD-214)
- U.S. individual taxpayer identification number (ITIN) authorization letter
- For residents 17 years of age or younger:
- School record or report card
- Clinic, doctor, or hospital record
- Day care or nursery school record
- A Missouri Department of Corrections ID (or equivalent from another state)
- Medical discharge papers or visit documentation
- St. Louis City Justice Center Wristband or another identifying document
- Utility bill with name and address of applicant
- Insurance policy (health, home, life, etc.)
- Medicaid/Medicare document or ID card
- Payroll stub that includes the last four digits of the applicant’s social security number
- Any other documents deemed appropriate by administrators
Proof of Residency - One Required
Any individual who has lived in the City of Saint Louis for at least 15 days can get a Gateway ID Card.
- Credit card bill or statement
- Voter registration card
- A utility bill (Ameren, Spire, MSD, City trash/water, cable TV service, Internet service, cellular service);
- A current residential property lease
- Local property tax statement dated within one year of the date of application
- Local real property mortgage payment receipt
- Bank account statement
- Proof that the applicant has a minor child currently enrolled in a school located within the city
- Employment pay stub
- Jury summons or court order issued by a federal or state court
- A federal, state, or local income tax or refund statement dated within one year of the date of application
- Insurance bill (homeowner, renter, health, life, or automobile insurance)
- Letter from government or social agency dated within one year of the date of application
- Missouri Dept. of Revenue form 5862, Certification of Homeless Status and Residency
- Written verification issued by a homeless shelter that receives city funding, confirming at least thirty days residency*
- Written verification issued by a hospital, health clinic or social services agency located within the City of St. Louis confirming at least 15 days residency*
*as specified by legislation
Applicants Under 18 and Individuals with Special Needs
- Applicants under the age of 18 and adults with special needs who have a legal custodian, caretaker, or guardian must be accompanied by their parent, legal guardian, caretaker, or legal custodian when applying.
- In addition, one document must be provided to verify the relationship or legal authority. Acceptable documentation includes:
- Birth certificate
- Adoption decree or certificate
- Birth certificate and stepparent marriage, civil union, or domestic partnership documentation
- Letter from a foster care agency or certification of licensed foster parent status
- U.S. or foreign court order establishing guardianship, custody, conservatorship, or power of attorney
- Verification of Release from the U.S. Department of Health and Human Services / Office of Refugee Resettlement (HHS/ORR)
- Letter from a licensed residential care facility or social service agency located in the City of St. Louis
- Social Security Administration letter establishing a caretaker as Representative Payee
- SSA letter establishing an organization as Representative Payee along with documentation authorizing the employee representative (employee ID required)
- Social Service Agency or foster care documentation may also be used to verify residency.
