2012 Weight Loss Challenge 2

Registration for the 2012 Weight Loss Challenge 2 is now open, but will end on Thursday, July 12. So please act quickly if you wish to participate.

The Challenge will now include individual prizes for men and women.  Please see the associated document for registration form, rules and tracking log.  

The Registration Form is a little different for this Challenge, but the information being requested is the same.  All team members should register on the Official Team Registration Form, indicating Team Name and where you are located.  All members must also sign the Registration Form.  Please return the completed Registration Form to Ryan Lord, Wellness Coordinator, at LordR@stlouis-mo.gov or by fax to 612-1488.  The deadline is Thursday, July 12, 2012.
