Request a Permanently Disabled Voter Application

A voter with a permanent disability may request to be placed on a list of voters who automatically receive an absentee ballot application for each election.

Overview

A voter with a permanent disability may request to be placed on a list of voters in City of St. Louis with permanent disabilities. Once you are on that list, you will automatically receive an absentee ballot application before each election in which you are eligible to participate.

 

Instructions

Download, fill out and mail the Permanently Disabled Voter Application. The application includes a page you can fold into an already addressed envelope, or you can use your own envelope and mail the application to:

What to Expect

After your application is received by the Board of Elections office, every time there is an election in which you are eligible to participate, you will automatically receive by mail an application for an absentee ballot.

You will need to fill out this application and return it to the Board of Elections, which will then mail you an absentee ballot.
