Community Health Improvement Plans

A community health improvement plan (or CHIP) is a long-term, systematic effort to address public health problems based on the results of community health assessment activities and the community health improvement process.

The Community Health Improvement Plan (CHIP) focuses on addressing the public health needs of residents of the St. Louis region. The CHIP builds on the progress and lessons learned from the first regional CHIP developed in 2019. Completion of both the CHA and the CHIP reflects a data-driven public health culture that values community direction and partnerships and requires a massive time-coordinated effort by all partners.

The City of St. Louis Department of Health and the St. Louis County Department of Public Health are, along with numerous partners, leading an effort to address health inequities and improve health outcomes for all residents in the region. This document reflects the concentrated effort of many organizations and individuals.
