Request an Inmate's Medical Records

Instructions to request medical records for a St. Louis City inmate. Medical records are kept for 10 years.

Overview

The Division of Corrections maintains inmate medical records for 10 years in accordance with state statute RSMO 109.255.

Preparation

  • Download and fill out the front and back of the Inmate Medical Records Request form.
  • Make sure to include an address and phone number on the front and sign and date the back of the form.

You can mail the filled-out form to the address below:

Or you may drop it off at the City Justice Center Monday-Friday 8:00 am-5:00 pm.

Fees

The Division of Corrections can provide a copy of records to former inmates free of charge, but we will charge for additional copies and copies sent to legal representatives, in accordance with state statute RSMO 191.227.

The Division can charge up to $25.34 for the record and $.58 per page. Once a record is invoiced, all checks or money orders for the record will need to be made out to the City of St Louis.

What to Expect

Records will be sent out as soon as possible, but depending on staffing, age of records, and workload it can take up to 30 days. Records will be sent in electronic format via email or fax to an individual or agency.
